Description

We are seeking a skilled and motivated Band 6 Community Occupational Therapist to join a Community General Rehabilitation team in Dartford, Kent. This role involves working closely with patients in a general rehabilitation ward and conducting home visits to support their recovery and enhance their independence.

Responsibilities include, but are not limited to:

* Conducting comprehensive assessments to identify patients' physical, emotional, and social needs.

* Developing and implementing tailored rehabilitation programs to achieve individual goals.

* Providing advice and support to patients, families, and caregivers.

* Performing home visits to evaluate living environments and recommend adaptations as needed.

* Collaborating with a multidisciplinary team to ensure high-quality, patient-centred care.

* Maintaining accurate and timely documentation of patient progress and interventions.

Requirements:

* HCPC registration and a degree in Occupational Therapy.

* Demonstrated experience in general rehabilitation or similar roles.

* Must hold a valid driver's license and have access to a car for home visits.

* Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.

* Willingness to complete an enhanced DBS if you do not currently hold one.

Pay rates:

* Day Weekdays: £30.54 per hour

* Day Saturdays: £33.15 per hour

* Day Sundays & Bank Holidays: £35.76 per hour
